Our employer group and Medicare Advantage plans include membership to Pro-Health & Fitness Centers, and as of January 1, 2012, membership at Parrish Health & Fitness Center.

Already a member of Pro-Health? If you're currently paying for a Pro-Health membership and become a HFHP member, be sure to update your Pro-Health account so you won't continue to be charged for your membership. Simply complete the hold form below and give it to Pro-Health (the form is also available from the front desk at Pro-Health). Changes will be effective the first day of the month after Pro-Health receives your form. You don't need to wait for your HFHP member ID card to make this change.

Preventive guidelines

As an HMO, we provide members access to preventive care. Our Preventive Care Guidelines for birth to 64 years (PDF) and for 65 years and older (PDF) are simply that, guidelines to follow for the majority of our members.
